- 01Inspect the device. Confirm the dose strength on the label matches the prescription. Eli Lilly's Zepbound Instructions for Use specifies that the solution be clear and colourless to slightly yellow, with no visible particles. A cloudy, discoloured, or particle-containing solution is grounds to discard the device.
- 02Clean the injection site. Swab the chosen site with the alcohol prep pad in a single circular motion, then let the alcohol air-dry for at least 10–15 seconds. Injecting through wet alcohol stings and can carry the alcohol into the subcutaneous tissue.
- 03Remove the base cap. Pull the gray base cap straight off the bottom of the device. The Instructions for Use specifies discarding the cap in regular trash, not in the sharps container, since it does not contain a needle.
- 04Place the device against the skin. Hold the device flat against the chosen injection site so the clear base sits flush against the skin. Eli Lilly's procedure describes a 90-degree angle to the skin surface. The device's hidden needle does not extend until the injection button is pressed.
- 05Press and hold the purple button. Press the purple injection button at the top of the device. The first click signals that the injection has started. Continue holding the device firmly against the skin without lifting.
- 06Wait for the second click. The Instructions for Use specifies holding the device in place until a second click is heard, which indicates the dose has been delivered. Eli Lilly recommends counting slowly to 10 after the first click as a fallback timer if the second click is not audible.
- 07Lift the device and inspect. Pull the device straight up off the skin. Confirm through the inspection window that the gray plunger has descended fully, this is the visual confirmation that the dose was delivered. A small drop of blood at the injection site is expected and not a concern.
- 08Dispose of the device. Drop the entire device into an FDA-cleared sharps container immediately. The Zepbound device is not reusable. Do not recap, do not bend, do not handle the needle.
Pre-injection checklist
- ✓Wash hands for at least 20 seconds with soap and water, or use an alcohol-based hand rub.
- ✓Check the carton expiry date and the device label match what the prescription says.
- ✓Inspect the solution through the inspection window, Eli Lilly's Instructions for Use describes the solution as clear and colourless to slightly yellow. Discard the device if the solution is cloudy, discoloured, or contains particles.
- ✓Allow the device to sit at room temperature for 30 minutes if it was just removed from the fridge, cold injections sting more and increase the chance of injection-site reactions.
- ✓Choose an injection site (abdomen at least 2 inches from the navel, the front of either thigh, or the back of either upper arm if a caregiver is administering). Rotate sites with each weekly dose.
After the injection
- Press a clean cotton ball or gauze square to the site for 10–15 seconds if there is any bleeding. Do not rub the site, which can increase bruising.
- Note the date, dose, and injection site (e.g. 'left abdomen, lower-outer') in a dosing log. Rotation is easier with a written record than from memory.
- Return the device's outer carton to a clean, dry storage area. Pen needle covers and the gray base cap go in regular trash; the device itself goes in sharps.
Common mistakes
- Severe abdominal pain that does not resolve, pancreatitis is listed in the Zepbound Important Safety Information.
- Difficulty breathing, swelling of the face, throat, or tongue, or hives, signs of serious allergic reaction.
- Severe vomiting or diarrhoea that prevents fluid intake, risk of dehydration and acute kidney injury per the Zepbound prescribing information.
- Severe upper-right abdominal pain, fever, or yellowing of the skin or eyes, possible gallbladder disease.
Frequently asked questions
Can I store the Zepbound pen in my car or gym bag for the day?+
Eli Lilly's Instructions for Use allows a single window of up to 21 days at room temperature, with a maximum of 86°F (30°C). A car interior in summer routinely exceeds 86°F and a single overheated day can require discarding the device. What if I do not hear the second click?+
Eli Lilly's Instructions for Use addresses this directly: count slowly to 10 after the first click as a fallback timer, then check the inspection window. If the gray plunger has descended fully, the dose was delivered. If the plunger has not moved, the device may not have engaged correctly, call the prescriber's office or Lilly's customer support line printed on the carton before re-attempting.
Why does the injection site itch hours after I injected?+
Mild itching, redness, or a small raised area at the injection site within the first 24 hours is described in Zepbound's prescribing information as a common injection-site reaction and typically resolves on its own within 1–3 days. A cool compress and avoiding scratching the site help. Where can I rotate among, exactly?+
Eli Lilly's Instructions for Use specifies abdomen (avoiding 2 inches around the navel), front of either thigh, and back of either upper arm (caregiver-only). With a single weekly dose, a four-quadrant abdomen rotation across four weeks plus alternating thighs every fifth and sixth week gives every site a six-week rest before reuse. Do I need to prime the Zepbound pen?+
No. Unlike multi-dose Ozempic and Wegovy pens which require a flow-check (priming) step before each new pen, the Zepbound device is a single-dose pen and is delivered already primed. The Instructions for Use does not include a priming step. Each device administers one full dose and is then discarded.
