Step-numbered Ozempic (semaglutide) pen administration procedure as described in the current Novo Nordisk prescribing information. For educational and patient-education purposes only. Not medical advice.
Key points
- 01Pre-use inspection. The pen is inspected for clarity and colour. The Ozempic prescribing information specifies that the solution be clear and colourless; a cloudy or coloured solution is described as grounds for discard.
- 02Needle attachment. A new single-use NovoFine or NovoTwist pen needle is attached to the pen as described in the Novo Nordisk Instructions for Use. Needle reuse is not supported by the manufacturer.
- 03Flow check (priming). Each new pen is primed before the first use. The dose selector is turned to the flow-check symbol and the dose button is pressed with the needle pointing up, until a drop of semaglutide appears at the needle tip, per the Instructions for Use.
- 04Dose selection. The dose selector is turned to the prescribed dose. The pen clicks once per dose increment. A mismatched dose is corrected by rotating the selector without pressing the dose button.
- 05Site selection and antisepsis. Abdomen, anterolateral thigh, or posterior upper arm are the three sites described in the prescribing information. The site is cleaned with 70% isopropyl alcohol and allowed to dry.
- 06Injection. The needle is inserted at 90 degrees and the dose button is pressed and held until the dose counter returns to zero. A six-second dwell is specified in the Instructions for Use to ensure complete dose delivery.
- 07Needle withdrawal and disposal. The needle is withdrawn and immediately placed into an FDA-cleared sharps container. The pen is stored with the needle detached, per storage guidance in the prescribing information.
- 08Post-use storage. In-use Ozempic pens are described in the prescribing information as storable at room temperature (below 30 °C) or refrigerated (2–8 °C) for up to 56 days. The pen cap is replaced after each use.
Frequently asked questions
What do the pen clicks correspond to?+
The Ozempic pen selector produces one mechanical click per dose increment as the selector is rotated. The Instructions for Use indicates that clicks heard while rotating the dose selector do not correspond to units delivered; delivery is indicated only by the dose counter returning to zero during injection.
How long is the dwell after plunger completion?+
A six-second dwell following the dose counter returning to zero is specified in the Novo Nordisk Instructions for Use. Early needle withdrawal is associated with incomplete dose delivery in the package-insert guidance.
What storage conditions are described in the prescribing information?+
Unopened pens are stored refrigerated (2–8 °C). In-use pens may be kept at room temperature below 30 °C or refrigerated for up to 56 days from first use, as described in the Ozempic prescribing information.
Is needle reuse supported?+
No. The Novo Nordisk Instructions for Use and the Ozempic prescribing information specify single-use needles. Reuse has been associated with needle-tip dulling and increased injection-site trauma in the published injection-technique literature (Frid et al. 2016).
What is the documented procedure for a missed dose?+
The Ozempic prescribing information describes the procedure for missed doses: if the next scheduled dose is more than 48 hours away, the missed dose is administered as soon as possible; if less than 48 hours away, the missed dose is skipped. Prescriber contact is indicated for uncertainty.
Is the semaglutide solution meant to be clear?+
Yes. The prescribing information specifies a clear and colourless solution. A cloudy, coloured, or particulate-containing solution is described as grounds for discard without administration.
What gauge and length needle is paired with Ozempic pens?+
The Ozempic Instructions for Use describes compatibility with NovoFine and NovoTwist disposable pen needles. Common paired specifications in the package-insert literature are 32 G × 4 mm or 31 G × 5 mm.
